CHILD Ultimate Bubble Soccer Injury Waiver Contract
Our sport activities (listed below) have inherent risks and can potentially cause injuries.
These injuries include but are not limited to:Ankle, Knee, Leg, Arm, Wrist, Hand, Ribs, Face, Head, Shoulder, Neck, Whiplash, and other unforeseen injuries.
The sport activities we run and organise include:
Notwithstanding any other clause in this Terms of Use, and for the avoidance of doubt, nothing in this Terms of Use is to be construed as releasing, indemnifying, limiting or discharging Ultimate Bubble Soccer and/or our directors, officers, servants, agents, invitees, or contractors (Personnel) from any liability caused or contributed to by Ultimate Bubble Soccer and/or our Personnel’s negligent or wilful acts or omissions. This clause will prevail to the extent of any inconsistency with any other clause in this Terms of Use
I, as the parent/legal guardian of , who will participate in the above listed activities at , On the date acknowledge the risks involved and potential injuries that may occur from his/her participation. I therefore take full responsibility for any injuries he/she may incur and I will not take any legal action against or seek financial compensation from Ultimate Bubble Soccer, Evgeni Routman and Ultimate Bubble Soccer staff
I understand that certain risks are inherent in the sport activities mentioned above in which my child will participate and I fully accept those risks. These risks include, but are not limited to, injury, and other physical harm to my child. I understand that there may be a great variety of other risks not known or reasonably foreseeable. I acknowledge that Ultimate Bubble Soccer, Evgeni Routman and other supervisors who run these sport activities are not responsible for any harm that may occur.
I confirm that my child DO NOT have any pre-existing injuries
I understand and agree that Ultimate Bubble Soccer, Evgeni Routman and other supervisors do not provide medical cover expenses for any injuries occurred.
I fully release and discharge Ultimate Bubble Soccer, Evgeni Routman and other supervisors from all liability in connection with my childs participation in these events/sport activities mentioned above.
